Abstract

Type 2 diabetes mellitus (T2DM) remains a significant public health concern, particularly in rural and underserved communities where patients often face barriers to effective self-management. This quality improvement (QI) project evaluated the impact of a diabetes intervention bundle consisting of individualized diabetes education, biweekly support phone calls, and digital self-monitoring tools on glycemic control, body weight, diabetes knowledge, medication adherence, diet compliance, and physical activity. The project was implemented over an eight-week period in a rural family medicine clinic and included 18 adults with uncontrolled T2DM (HgbA1c >7%). Participant outcomes were evaluated using pre- and post-intervention clinical measures, the Diabetes Knowledge Questionnaire (DKQ-24), participant logs, and digital health records. Following the intervention, participants demonstrated improvements in glycemic control, body weight, medication adherence, diet compliance, physical activity, and diabetes knowledge. Although the digital applications showed mixed success because some participants preferred using their existing continuous glucose monitor (CGM) applications or had trouble navigating smartphone technology, the education sessions and follow-up phone calls were well received and supported patient engagement. These findings suggest that individualized education combined with ongoing support is a practical and sustainable strategy for improving diabetes self-management in a rural primary care setting.
