Abstract
Background: Breast cancer remains a leading cause of cancer-related mortality among women in the United States. Despite the effectiveness of mammography in early detection, Hispanic women in rural communities experience lower screening rates due to socioeconomic, cultural, and access-related barriers. At a rural South Texas clinic, the baseline screening rate among Hispanic women aged 50–74 years was 70.6%, below national benchmarks.
Purpose: To increase breast cancer screening compliance among Hispanic women aged 50–74 years in a rural South Texas family practice clinic.
Methods: Guided by the Lean A3 quality improvement framework, this project implemented a multifaceted intervention bundle that included EMR screening alerts, staff education, bilingual patient education, reminder letters, follow-up outreach, motivational interviewing, and collaboration with local imaging services. Outcome measures included screening compliance, patient knowledge scores, and reported barriers to screening.
Results: Screening compliance rose to 87.02%, surpassing the baseline and national benchmark. All participants (100%) received breast cancer screening education. Knowledge scores increased from 7.28 pre-intervention to 9.40 post-intervention. A paired t-test showed a significant increase in knowledge, t(44) = 19.00, p < .001, with a large effect size (d = 2.832).
Conclusion: A culturally tailored, evidence-based intervention improved breast cancer screening and patient knowledge among Hispanic women in a rural primary care setting. These findings support the use of structured quality improvement strategies to reduce screening disparities and improve preventive healthcare in underserved populations.
