Abstract

Objective: Implementing a safety bundle checklist to provide interventions for the modifiable risk factors identified in patients scheduled for colorectal surgery can lower the risk of surgical site infections (SSIs) and costs for both the patient and the organization.

Methods: This was a quality improvement project that utilized the implementation of the surgical patient safety system (SURPASS) bundle in conjunction with the World Health Organization (WHO) surgical safety checklist. Patients who were inpatient and scheduled for colorectal surgery (elective and non-elective) were identified; colorectal surgeons, anesthesia providers, hospitalists, and inpatient staff were provided with education on the SURPASS bundle. Pre- and post-interventional data was collected on surgical site infections, surgery on-time starts, and missed labs and orders from the electronic health records; 8 weeks pre-intervention compared to 8 weeks post-intervention. A statistical analysis of the pre- and post-interventional data was completed.

Results: The total number of colorectal cases during the 8 weeks pre-intervention and 8 weeks post-intervention was N = 107; the pre-interventional case number was n = 45, and the post-interventional case number was n = 62. Pre-intervention, 1 SSI occurred, and post-intervention, 1 SSI occurred. Preoperative data showed that the average surgery delay time was 17.96 minutes compared to the post-intervention delay time, which was 22.78 minutes.

Conclusion: The SURPASS bundle is a valuable tool to standardize the process of inpatient readiness for surgery with appropriate enhancements to educate the nursing staff to improve understanding properly.
