Abstract

Obesity can cause multiple chronic conditions, including cancer and death. Los Angeles jail primary care clinics do not address obesity management despite health hazards, rising costs, and high obesity rates in correctional facilities. The Centers for Medicare and Medical Services (CMS) obesity-management guidelines are over ten years old and outdated. The aim of this Doctor of Nursing Practice (DNP) project was to update the CMS obesity-management guideline with the newest intense behavioral therapy (IBT) evidence to assist the correctional multidisciplinary healthcare team. This project took place in a Los Angeles correctional facility's primary care clinic. A detailed literature review gathered current and relevant literature, which was subsequently appraised and graded using the Johns Hopkins Nursing Evidence-Based Practice (JHNEBP) Evidence Level and Quality Guide. Ten recommendations were identified and graded based on the evidence-based literature. Question Pro software was used to rate the recommendations by the stakeholders compromising, primary care clinicians, nurse educators, and mental health providers treating obese jailed male adults. A Friedman’s two-way ANOVA by rank, followed by the Chi-Square test, was used to calculate differences between the stakeholders’ recommendation ratings. The finalized obesity-management guideline for the correctional setting included the ten recommendations, each of which received a score of three or above, indicating its strength and reliability.
