Abstract
Abstract
Background: In the United States (U.S.), falls are the leading cause of injury among adults 65 and over, resulting in 36 million falls yearly (Moreland et al., 2020). According to the Centers for Disease Control and Prevention (CDC, 2023), one in four older adults experiences a fall each year. Falls are the world's second most prominent cause of accidental deaths (World Health Organization [WHO], 2021). Falls are the leading cause of both fatal and non-fatal injuries among older adults (Moreland et al., 2020).
Methods: A quality improvement project that included a fall bundle was implemented in a primary clinic. A retrospective chart review was performed utilizing the clinic’s electronic health record to collect pre-intervention fall occurrences. Data was gathered over eight weeks on 100 patients over the age of 65 who met the inclusion criteria. Descriptive statistics were performed on the demographic variables, including age, gender, race, and fall history.
Results: The number of falls decreased significantly from eleven occurrences over eight weeks before the intervention to just one incident post-intervention. These findings strongly support the clinic's decision to implement routine fall risk assessments at every outpatient visit as part of its fall prevention protocol, showing measurable effectiveness in reducing the incidence of falls among our older adult patient population.
Conclusion: The considerable decrease in fall incidents, from eleven falls eight weeks before the intervention to just one fall following the intervention, underscores the need to conduct routine fall risk assessments during every outpatient visit. These results provide compelling evidence of the measurable impact of reducing falls among our older adult patient population, emphasizing the need for proactive assessment in outpatient primary care settings.
