Abstract
Abstract
Background: The prevalence of depression poses a significant challenge in primary care, emphasizing the urgent need to address this issue during patient visits. Primary care providers are at the forefront of managing depression, yet they encounter difficulties in screening, diagnosing, assessing, and treating this complex condition. Clinical guidelines serve as valuable tools for primary care providers and practices, offering support in identifying and addressing depression effectively. An evidence-based approach to treating depression in primary care, with a focus on collaborative interventions with behavioral health to enhance depression management for American Indian/Alaskan Native (AI/AN) communities.
Methods: The descriptive analysis was conducted for managing depression in primary care by grading 88 literature reviews, ranked recommendations, and stakeholder ratings. Descriptive statistics were calculated using a correlation analysis using the Friedman rank-order correlation coefficient. This analysis evaluated how stakeholders ranked the recommendations similarly or differently, utilizing a rating scale of one to four to determine the correlation between stakeholder ratings. The correlation indicated the extent to which stakeholders gave the same rating to each recommendation. The project utilized the Friedman analysis technique to determine if there were any statistically significant differences between the stakeholders' choices of 1-4 ratings.
Results: The descriptive statistics and Friedman's two-way ANOVA by ranked data showed that the test statistic had a Chi-square value of zero and three (3) degrees of freedom, with a significant level of p=1.00. The results indicate that there is no substantial difference or significant difference among the ratings. All recommendation ratings are uniformly excellent, with a score of "4" across all stakeholders.
Conclusion: The stakeholders rated all four recommendations and unanimously ranked them as excellent, signaling their full approval. The overwhelming approval resulted in outstanding ratings for all the recommendations. The project analysis revealed no statistically significant difference in the ratings, underscoring a solid stakeholder consensus. The clinical practitioners overwhelmingly advocated for implementing clinical practice guidelines, explicitly endorsing assessment tools such as PHQ9, Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5), severity assessment, and collaborative treatment approaches, emphasizing the importance of a team-based approach.
