Abstract

Type 2 diabetes requires consistent self-management support to promote glycemic control and reduce complications. This eight-week quality improvement project implemented a standardized diabetes self-management care bundle for adults with type 2 diabetes and hemoglobin A1c greater than 7% in a nonprofit primary care setting. The quality improvement project was guided by the Plan-Do-Study-Act framework. The intervention incorporated self-monitoring of blood glucose (SMBG) review and provider feedback, medication adherence assessment, prescription review in the patient’s preferred language, diabetes self-management education and support reinforcement, nutrition counseling or dietitian referral, and follow-up planning. Thirty patients were consecutively enrolled. Following the implementation, medication adherence assessment and diabetic self-management education and support (DSMES) reinforcement were documented for 100% of participants, preferred-language prescription review and follow-up documentation for 96.7%, nutrition counseling or referral for 76.7%, and SMBG review with provider feedback for 96.7%. Among participants with complete paired fasting-glucose data (n = 29), fasting glucose decreased from 161.38 mg/dL (SD = 44.67) to 133.59 mg/dL (SD = 26.63), t (28) = 5.288, p < .001, with a large effect size (Cohen’s d = .98). The findings support continuing the standardized diabetes self-management care bundle as part of routine primary care while continuing to evaluate its effectiveness and sustainability.
